NAME: Grade (circle one): 9^th 10^th 11^th

Place a tally next to each category that applies to you. Only include achievements/activities from THIS YEAR. Activities may not be counted until they are completed/performed. Do not include events that have not yet happened. You may place multiple tallies next to an activity you have done more than once (i.e. singing at games, performing in school musicals). This is an “honor system”, please be honest.

To be eligible for a choir letter, you must have spent at least one full year in RRHS Choir, and you must be registered for choir in the next school year. Failure to participate in the choir program will result in the removal of this honor.
